UST SYSTEM: Release Detection: Tanks: Electronic monitoring of tank interstices Piping: Electronic monitoring of piping interstice and mechanical line leak detector with annual 3 gph leak tests. Visual inspections of dispenser hoses and spill containment buckets (including interstice). Veeder-Root TLS 350 panel (DEP EQ # 197) checked on site - All functions normal. Audible/visual alarm checked - functional Note: Electronic sensors are located in the STP Sumps and dispensers but are not used as part of the release detection per Release Detection Response Level Statement (RDRL) Tank interstitial alarm history report on file Tank: Underground System: (2) 20,000- gallon, Modern Welding Glasteel II, double-walled, fiberglass-clad USTs (DEP EQ # 156). (1) UST is compartmented 15,000-gallon (Regular) and 5,000-gallon (Premium); the second UST contains Diesel. Underground piping is A. O. Smith Red Thread IIA, double walled, fiberglass piping (DEP EQ # 252). Tanks equipped with: OPW STP sumps ¿ All appeared intact and dry; except for the premium sump. A ½ inch of fuel was noted in sump, below penetrations. There were no obvious leaks; need to remove fuel and check STP and piping for leaks. Need to file and Incident Notification form and investigate the cause of the leak. Provide to EPC office within 24 hours the INF form and within 14 days provide the result of the investigation. Red Jacket STPs with mechanical in-line leak detectors present. No rust on STP heads. Veeder-Root electronic sensors present; positioned correctly (although not used as release detection per the RDRL). Secondary piping test boots appear open into the sumps. Emco Wheaton, DW spill containment buckets checked; need to pump liquid out of the premium and regular unleaded primary buckets. The diesel full bucket had fuel in the interstice. Need to remove fuel from interstice. Need to file and Incident Notification form and investigate the cause of the fuel in the interstice, repair and test double wall spill bucket. Provide to EPC office within 24 hours the INF form and within 14 days provide the result of the investigation. Fill covers were properly color coded Co-axial vapor recovery present (Regular & Premium only, none for Diesel); Overfill protection - (3) OPW 61SO flapper valves present in fill tubes; Vent lines with caps Dispensers: (6) Dispensers checked. OPW liners (gasoline dispensers); appeared dry and intact and (2) OPW Flexworks liners (diesel dispensers); appeared dry and intact. Dispenser # 8 and 9 had approximately 1/2 inch of diesel fuel in liner, below penetrations. There were no obvious signs of leaks. Need to remove fuel from dispenser liners and investigate the cause of the leak. Need to file an Incident Notification form and complete investigation within 14 days. Need to remove water from dispenser liners at dispenser #3, above penetration. Shear valves have anchors Secondary piping interstitial ports are closed to liners Hoses/Nozzles ¿ okay and intact. No evidence of any leaks. The five year Breach of integrity tests on underground storage tanks were performed on 04/03/2013 by Central Florida Tank testing. Tests passed, next tests are due by 04-03-2018.

Description: On 08/08/2013@1115; MLM- met with Mike Pond on site for the Annual Compliance Inspection. Storage tank system had been taken out of service. Tanks were emptied to less than one inch of fuel, fills were locked, vents open and dispensers were secured. Double wall storage tank system can remain out of service for 10 years. Received STRF for out of service on 08/15/2013 and STRF was submitted to FDEP on 08/15/13. Release Detection: Tanks: Electronic monitoring of tank interstice Piping: Visual monitoring of piping interstice and mechanical line leak detectors with annual 3 gph leak tests. Visual inspections of STP sumps dispensers/liners and double walled spill containment buckets with interstice. Note: Electronic sensors are also located in the STP sumps but are not used as part of the release detection per the Release Detection Response Level (RDRL) statement. Veeder Root TLS 350 panel (EQ # 197) with sensors (EQ# 614) checked - All functions normal Tanks/STP Sumps/Piping: (2) 15,000-gallon, Xerxes, double-walled USTs {(1) 15,000-gallon UST compartmented (Premium and Diesel) (1) 15,000 regular gas}. Underground piping is A. O. Smith Red Thread IIA, double walled, fiberglass piping (DEP EQ # 252). Tanks equipped with: (2) STP sumps ¿ Dry and intact. Red Jacket FXIV Mechanical leak detectors present (EQ# 373) Secondary piping test boots appear open. (2) Double walled spill buckets OPW 700L (EQ# 640) ¿ Intact and dry. Fill covers properly color coded Overfill protection - OPW 61SO flow shut off valves present in fills (EQ# 223); Co-axial vapor recovery for Premium & Regular fills (none for Diesel). Vent lines properly vented and capped. Dispensers: (4) Dispensers liners and shear valves were not checked. Records reviewed: Current registration Placard: (2) tanks. Written Release Detection Response Level Statement: Present, complete and accurate Monthly release detection monitoring records reviewed include: Visual inspection of STP sumps, dispensers/liners and spill containment buckets with interstice Audible/visual alarm checked, okay. Sensor status check and printouts. Veeder Root sensor printout on 06/04/2013 should a sensor alarm on the regular tank interstitial. Provide an incident Notification form within 24 hour and perform an investigation of the sensor alarm prior to placing storage tank back into service. Also on 07/08/2013 Veeder Root panel was not working. Need to investigate and repair the problem with Veeder Root panel. Inspections performed within 35 days Sensor alarm history report was not available. Leak detector tests and operability tests were due by 06/20/13, but not performed because tanks had been taken out of service. Breach of integrity tests on tanks, leak detector tests and sensor operability test must be performed prior to placing storage tank system back into service.
