The header of the inspection report was modified to state "Storage Tank Facility Closure / Installation Site Inspection". In both FIRST and OCULUS there are separate inspection reports. In addition, failure to test the line leak detector was cited as a SNC A violation (62-761.610(1)(b)) when it should have been a SNC B violation (62-761.600(1)(i)).

The last test was completed on 8/22/2005 and the tests that were due 8/22/2006 and 8/22/2007 are overdue. The operability test verifies that the line leak detectors meet the performance standards of Rule 62-761.640(4)(a), F.A.C. (#1067 / #1074) 2. Alarm history tapes do not confirm that an annual operability test of the Veeder-Root sensors has been conducted prior to 3/20/2007. The last test was completed on 3/20/2006. (#1074 / #1117) 3. Alarm history tapes indicate that 14 liquid level sensor alarms have not been properly documented, investigated, and resolved. These are: L1 ¿ Alarm on 11/27/2006. L2 ¿ Alarms on 10/2/2007 and 9/28/2007. L3 ¿ Alarms on 9/27/2007 and 5/18/2006. L10 ¿ Alarm on 4/18/2005. L11 ¿ Alarm on 6/14/2006. L12 ¿ Alarm on 4/18/2005. L13 ¿ Alarms on 4/18/2005 and 9/24/2004. L14 ¿ Alarm on 12/10/2007. L15 ¿ Alarm on 3/15/2006 L16 ¿ Alarm on 3/15/2006 It would appear that the alarms need to be investigated and the entire system inspected for deficiencies.
