Description: Meaghan Godwin Final Installation Inspection On-site at 12:15 P.M. with Mrs. Hilda Enriquez with Westar, Manny Cordero, Project Manager, and Coraliz Cordero from Kinetik Industrial Group, LLC (PCC #: 1256903, (561) 231-1203) doing business as Fuel Services Management, Inc. for the final installation inspection of (3) New Metal Products (EQ-333) double walled, underground storage tanks (USTs): One (1) 10,000-gallon storing regular gasoline, one (1) 8,000 gallon compartmentalized to store 4,000-gallon premium gasoline and 4,000-gallon diesel, and one (1) 6,000-gallon compartmentalized to store 3,000-gallon E-85 and 3,000-gallon RED-90, as per plans. The DERM Approved Plans dated 01/29/2020 were on-site and coincided with the site layout. The requested surveys were provided. The current FDEP Registration Placard was available on-site. The Certificate of Financial Responsibility was current and available. The policy is being held with Crum & Forster Special Insurance Company and is running from 02/16/2020-02/16/2021. All the required tests were provided and had passing Test results. The Line Tightness Test, Annual Inline Leak Detector Test, Overfill Protection Test, Release Detection Test, and Breach of Integrity Tests were done by PSR, Inc. on 09/02/2020 and 09/11/2020 and passed. Electronic release detection is monitored through the Franklin Fueling Systems EVO-550. All functions were normal at the time of the inspection. The liquid status report was conducted. The sensors monitor the five (5) STP sumps, three (3) Annular Spaces, and three (3) dispensers. No alarms noted at the time of inspection. The visual and audio alarms appeared to be working properly. There are a total of three (3) dispensers on site. Dispensers’ 1-2 and 5-6 liners were dry, dispenser 3-4 contained product that was removed and the components checked for leaks. The shear valves appeared to be properly braced. Hoses and nozzles were intact. Fill port & vapor recovery lids were properly color-coded. Fill port & vapor recovery watertight caps contained gaskets and had tight seals. Fill port & vapor recovery spill containment buckets were dry. All the STP sumps were dry and contain mechanical line leak detectors. No corrosion noted on STP components. The piping boots were pulled back for release detection and the sensors were correctly positioned below piping. There were a total of (6) monitoring wells on site. The Approved Plans only call for 5 monitoring wells. All monitoring well concrete pads and lids were intact and properly color coded. All monitoring well watertight caps were locked or properly secured and contain gasket. All monitoring well collars/risers were at least 1 inch above the surface and the containment appears to be properly grouted. No visual evidence of free floating product or sheen observed, as well as no odors were detected during the inspection in all monitoring wells. All monitoring wells DTW/D was measured on 09/10/2020. On that date water column levels were determined adequate for release detection meeting the 3 ft. requirement. Class A Operator Training Certificate was available. Class C will be taught to new employees. A site diagram was drawn and pictures were taken. The Lat/Long coordinates have been updated on the STCM-Map. The facility has passed the Final Inspection. Description: East Tank Farm (5) Vertical, single walled, steel ASTs. Tanks are located within a sealed concrete containment area. Interior of containment area measured: 50' x 68' x 3' (76,296 gallons). Containment area has been sealed with PPG Industries sealant DEP EQ-455 No problems noted with integrity of the concrete. Slight surface cracking noted near top of wall, but volume of containment is well above 110% of the regulated tank). No drains in secondary containment, rainwater removed with a pump according to operator. An additional coat of sealant will be added according to Manuel Sequera. Unregulated tanks within containment area: - Tank # T1E - Liquid Asphalt - Tank # T2E - Liquid Asphalt - Tank # T3E - Liquid Asphalt - Tank # T4E - Empty Regulated tank within containment area: - Tank # T5E - 20,000 gallon single walled steel AST containing mineral spirits. No tank equipment approval number, but tank was certified by a professional engineer form TEAM Consultants, Inc. indicating the tank meets/exceeds UL142 standards. Tank T5E is only used to fill Tank #T6. Tank #T5E is not a flow through process tank because it is used for the storage of regulated substance before its introduction into the process. There is not a steady, variable, recurring, or intermittent flow of materials during the operation of the process. Release Detection: Visual inspection of storage tank system components including secondary containment area. Tank # T5E is equipped with: -Exterior coating. -Product labels at remote fills. -Normal venting and emergency venting (according to PE certification). (2) Remote fills. -Remote fill within containment area and within a spill containment bucket. Spill containment drain valve closed. This fill is not usually used according to operator. Tank is mainly filled by railcar according to operator. Remote fill at railcar unloading area has no spill containment (not required, rail car unloading operations exempt). Remote fill from rail car unloading area is coated, aboveground steel piping Supply line is approximately 160 feet of coated, aboveground, steel piping which runs through the warehouse, then to Tank #T6. Supply line is equipped with a manual isolation valve at tank outlet. Supply line is not equipped with an anti-siphon valve. Installation application indicates piping is 4 inch, but piping through warehouse appears smaller. Antisiphon device required for small diameter piping. Forwarding pump located within containment area in order to pump mineral spirits to Tank #T6. Piping has been repainted since last inspection. Very slight rust noted on piping before entry into warehouse, to be repainted according to operator. Tanker loading area north of containment (tight fill fitting off the supply line to Tank #T6). Not often used according to operator (tanker loading operations exempt). -Overfill protection - Concrete secondary containment. Fuel level sight gauge (unknown make/model) is present, but is not visible from railcar unloading area. -Recommend electrical grounding. No leakage noted. Other unregulated tanks on site: Tank #T7 Stormwater Tank #T8 Spinks (clay) slurry

Description: A thirty-day late payment letter was issued to Mr. Thomas. The final payment of $1,200.00 was due by January 24, 2010 but has not been received by the Department. Also, the August 2009 to October 2009 monthly release detection inspection records and CSLD printouts for the Regular UST were to be submitted to the Department as they were completed. However, these records have not been submited to the Department as required per the executed Model Consent Order (OGC #09-3010). Mr. Thomas was advised to submit the payment and records by March 16, 2010.
