| Message: | Good Afternoon, I am contacting you to inform you of an FDEP Aboveground Storage Tank Compliance Inspection for 9201872 St. Johns County Public Works Facility at 2750 Industry Center Road, 1625 Hwy 16, Saint Augustine, FL 32084. I would like to schedule your inspection for Friday March 12 @ 10:00 AM. Since the last inspection was conducted on December 11, 2018, I will need to review the Financial Responsibility (CFR) forms, annual testing, monthly inspection records, and current Placard for the timeframe covering January 2019 up to March 2021. Here is a breakdown of the paperwork we would like to review prior to or during the inspection. New regulatory rules stated: Any documentation produced on January 11, 2017 and going forward will need to be kept for three years. Records to be reviewed: Due to current circumstance and social distancing protocols, all records will be requested prior to the inspection. Under further investigation it was determined that the entry boots and electrical conduit boots inside of dispenser 5/6 and 7/8 are compromised and need to be replaced. On 01/06/2016 Guardian Fueling Technologies removed all the old rubber entry and electrical conduit boots inside of dispenser 5/6 and dispenser 7/8. To be able to access the old boots and install new ones the concrete one each side of each dispenser had to be removed. | Description: | Good morning Mr. Carter, A storage tank compliance inspection has been scheduled for Tesnic LLC located at 6401-A Harney Rd in Tampa for November 17th @ 10:00. During the compliance inspection I will be required to review the following documentation: -Third Party Pollution Liability Insurance Policy for 2014-2015 and 2015-2016. -Certificate of Financial Responsibility for 2015-2016. (I have attached a blank copy to this email). -Current Storage Tank Registration placard. -Written Release Detection Response Level statement. (I have attached a blank copy). -Monthly visual inspection logs from June 2014 to present. If you would like help filling out the Written Release Detection Response Level statement and the Certificate of Financial Responsibility please have them printed out and on site. I will assist you in filling them out during the inspection. The past compliance inspections at this facility have noted missing documentation, such as the monthly monitoring logs, and the storage tank registration placard. There was also a past violation for leaves built up underneath the aboveground piping.
