Description: 08/13/2019 CRW/TCI Met onsite with Drew Dupree of Grand Prix Tampa for the routine compliance inspection of the tank system and components. Release Detection: -Visual inspections of the tank system and components. -Visual inspections of the Krueger At-A-Glance Leak Gauge -Krueger At-A-Glance Leak Gauge appeared to be in normal position. Tank: (1) 2,000 gallon double-walled Convault aboveground storage tank containing gasoline for vehicular use. Tank is located on a concrete pad and is equipped with: -Product label: Gasoline. -Normal and emergency venting. -Top mounted spill bucket: appeared to be dry and intact. -Overfill Protection: Krueger Fuel Level Sight Gauge. -Aboveground, single-walled, coated, steel piping runs from tank to side mounted Fill-rite dispensing unit. -Anti-siphon valve is located upstream and adjacent to the manual isolation valve. Aboveground tank systems equipped with aboveground piping that produces a gravity head must be equipped with a manual isolation valve upstream and adjacent to the anti-siphon valve. In the event of construction to the piping, correct the order of the valves and contact EPC for an inspection 30-45 days and 48-72 hours prior to initiation of closure and installation activities. -Tank exterior appeared to be in good condition. -Electrical grounding present. No obvious signs of leakage noted. Dispensers: -(1) Side-mounted Fill-rite dispensing unit inspected: appeared to be in good condition. -Hoses/nozzles appeared in good condition. No obvious signs of leakage noted. Records: -Current registration placard posted onsite: (1) tank. -Monthly monitoring logs reviewed and include: Visual inspections of the tank system and components. Visual inspections of the Krueger At-A-Glance Leak Gauge. No problems noted. Inspections completed within 35 days. Reviewed inspection results onsite with operator Drew Dupree of Grand Prix Tampa.

Description: This office follow up was conducted to close the open violation cited during the annual compliance inspection done on 2/2/2017. The open violation was the following: Complete an incident notification form investigating the cause of the regulated substance inside the premium STP sump. Submit form to Cabrev@miamidade.gov An incident notification form was received via email. The incident has been recorded in FIRST and did not lead to a discharge. The system tests information has been updated in FIRST. The annual operability and line leak detector test were done by Discovery Tank Testing on 5-9-2017. The open violation has now been resolved, facility is now in compliance. There is no representative signature as the report was completed as an office follow-up. A copy of the inspection report will be sent via email to the station representative Mrs. Ibon Silva at isilva@stationsmaint.com for facility records. The electronic communication will be added to FIRST.

This was reported as a ''vapor lock" issue by the delivery driver. •:+ Circle K received additional information from the store team that a small amount of product may have come out of the vapor port. •>Circle K store employee, Sharonica, working on site May 41h, stated she called her manager and Market Manager regarding a fuel release. +:• On this date, Sharonica also took some pictures of v.ttat she thought was water but then determined that she thought it was fuel. (There was a communication breakdown here, as this documentation was not mentioned or provided until May 121h.) + On May 4th Guardian Fueling Technologies was dispatched to investigate. •>Guardian reported no fuel in the fill bucket and a small amount of fuel in the vapor bucket. + Guardian stated there was no fuel on the concrete nor any visible product anyv.ttere outside the vapor bucket. +Guardian replaced an STP check valve and probe on May 4th and sth. VVhen they replaced the probe, the fuel volume reading was 93". A High Product Alarm registers on probe installation, May 4th at 5:36 PM. May g1h _ •:+ Circle K received mixed feedback regarding the May 4th incident cause. +The Circle K TSM, David, indicated there was a carrier/delivery overfill on May 4th. He stated this information came from Guardian . +:• Circle K requested ECS to review on-site conditions as a precaution. This was scheduled for Monday, the 11th. May 11th - +:• ECS was on-site to re-evaluate and verify on-6ite conditions. •> ECS detennined a discharge may have occurred. + A ORF was filed. May 12th- +Circle K received a call from Richard Orth regarding follow-up on a spill and potential tank overfill. •:+ Sharonica (store employee) was contacted, and she staled there was a canier overfill. She stated the fuel driver asked her to hit the e-stop. +:•Pictures were communicated to Heather Gilmore (Circle K Tampa office) on May 13th. The pictures show what appears to be fuel that had run into the grassy area by the fill. + Eagle (fuel canier) states there was no overfill or release incident on that date - only a vapor lock issue. +:• The Tank 2 probe was broken so there is no delivery infonnation for that tank in the ATG. •> The canier reported the fuel level at n" on his departure. + Circle K confinned 'W'ith ECS that 3-4 drums of soil were excavated between the fill/vapor connection and the vent stack. + ECS confinns a recent discharge - no free product - and they are comfortable stating that <25 gallons were released. •:+ Based on OVA readings and the soil excavation area, ECS believes that fuel had come from the area of the fill/vapor connection and not out of the vent stacks. + ECS pe1fonned an initial source removal: they pulled three soil samples and one groundwater sample. Results should be available in 7-10 days. +Circle K shows no fuel inventory loss: the store papel'\Wrk is a little sloppy Ot lags) with regard to logging deliveries. This has been addressed at the site level. The incident cause is believed to be a combination of driver error (no vent or overfill prevention issues \'Yere found) and possibly a bad check valve on one of the Regular tank STPs, allowing fuel from the product lines to be pumped back into the second Regular tank as fuel dispensed.
