Description: Inspection 01/08/2009 for removal of damaged SW OPW spill containment bucket at Regular Unleaded gasoline tank fill, the northern of two tanks linked by a siphon bar. The other fill has had its spill containment replaced previously. The flow shutoff from the fill was found to be defective by the conrtractor, Superior Pump Services, Marcus Hart, PC 1256834, and it is also being replaced. Samples of soil arounf the removed fill were taken by EnviroProTech, who will provide a Closure Assessment within sixty days, not later than 03/09/2009. This site is EDI-eligible from the first of three previous discharges, and a Limited Summary CAR will suffice. The excavation had a fairly strong product odor.The removed bucket had two large piercings on opposite sides of the elastomeric bellows: see attached photos. No registration update will be required. -JSS

Sumps for Submersible Turbine Pumps and Diesel siphon piping are Total Containment PE (EQ-295.) Release Detection is Simmons SIR 5.7.L.M. (EQ-624) with a tank tightness test every three years. Inventory data is provided by a Veeder-Root TLS-350 Automatic Tank Gauge (EQ-180.) Description: AST #4 is a DW Steel Generator Sub-base Tank by Custom Tank Technology, Model UCTCT-750, (Ul-142 certified but NOT on FL Approved Equipment List) carrying an Onan Genset. No external piping. No Sumps. Release Detection for these ASTs is an electronic alarm triggered by a liquid sensor in the rupture basin. These alarms are monitored remotely at a central system in the control room. Each was modified in May of 2003 to include a test switch at the west end of the genset to trigger the alarm , for monthly monitoring and annual testing, by Coastal Generators, Inc.

Description: EXPLANATION for REPORT & LETTER TARDINESS: Facility inspection occurred on 5/18/2012 prior to memorial day weekend holiday week. Upon returning from vacation our Department had been surprisingly dissolved. My director resigned and my supervisor was no longer working for the County. Therefore, attention was required for internal stability during the transition. Then, when attempting to complete the inspection data entry collected during the physical inspections, the FIRST system crashed on my computer in late June. I could could not receive support due to the FIRST support contract being put out for bid and the former support contractors leaving. My toughbook for FIRST was not functional until the second week of July. This inspection was previously entered in disconnected mode but disappeared due to the computer issue. Therefore, the violations, insurance, and records checks were entered in a new inspection report, but the fully typed inspection report and letter were scanned and attached that disappeared during the computer issue. ********VIOLATION RESOLUTION GRACE PERIOD********** The facility is being granted a grace period for corrective action until August 15, 2012 for violation resolution due to the issues described above.
