Inspector unable to identify the parts in use in the spill bucket. Owner indicated that he had plugged spill bucket. Unclear if secondary monitoring is available. Email received from owner with a picture of a clean spill bucket. Owner indicated that the spill bucket had been cleaned and hydrostatically tested and found to have integrity. Owner did not identify the parts used or if secondary monitoring was still functioning. Attached pictures of the spill bucket showing progress from Routine, Re-inspection, Owner picture. Inspector was unable to remain at the facility for the duration of the inspection but did received conformation letter (attached) from the foam contractor, stating that the filling of all 3 tanks had been completed. Description: Teresa Rahrig (TR-SED) met with Hector Parades (HP-Miami-Dade) on site to inspect the Miami Dade Cnty Fleet Mngt. 3 B storage tanks. The FIRST database reports (3) three double walled fiberglass storage tanks registered at this location including two vehicular diesel 12,000 gallon unleaded underground tanks along with one unleaded 12, 000 gallon tanks. Registration information appears to be correct. Note that all responsible parties have not been identified at this facility. Facility owner/operator and property owner have not been identified. A state registration form should be submitted to update database information. All applicable records have been provided and reviewed as indicated in this inspection. Note that the VR operability test results indicates only 2 STP sump sensors, whereas the VR panel indicates 3 STP sump sensors. The tanks are registered as double walled USTS for fueling county motor vechicles. There was no evidence of staining or leaks. Spill buckets and sumps were intact and free of product or water. Overfill protection is provided by an alarm with the Veeder Root TLS 350. Vent pipes are present and intact. The interstitial space is continuously monitored by the Veeder Root TLS 350, as well as the piping sumps. I could not locate the sensors in the sumps, however, a printout of the sensor report indicates L5-L7 STP sensors and L1-3 Annular sensors normal. It is recommended to have the sensor location in the sumps be verified as being located at the bottom of the sump, as recommended by Veeder Root. It was noted that T 1-T 3 probes recorded pass .2 gallon/hr tests. The VR Panel had no alarms. Piping appears intact and in good condition in the sump, with no evidence of a discharge. I could not locate the sensors on the bottom of the sump, they need to be verified that they are located at the bottom per direction Veeder Root Operations manual. All dispenser liners appear to be dry with no evidence of a discharge, the dispenser pan bottom is intact and dry. Shear valves are properly braced and anchored. Follow-up: The underground tank systems are in compliance for this facility. A signature was not obtained during the inspection due to FIRST issues, but an electronic copy will be mailed to Hector Paredes within one week of the inspection. Photos were taken of the dispenser, sump and tank pad. A site sketch was redrawn to indicate tank locations for the FIRST database.

Met Mr. Scott McElhaney at facility. Current registration is available. Registration information reviewed with Mr. McElhaney. Insurance documentation and current Certification of Financial Responsibility form provided by Mr. Greer. Release Detection Response Level description example provided. Monthly inspection log is current and timely. Log does not note monthly alarm circuit testing. Site plan sketched and new file photos taken. Lat Lon info reviewed in SWAP. For double wall storage tank systems in which the interstitial space (rupture basin/ double wall space) is monitored electronically, facility must test the alarm circuit monthly to determine if the alarm is functional. The Alarm switch must be physically tested annually to verify the switch is functional and will put the system into alarm. Manual monthly interstitial monitoring is an acceptable alternative to electronic monitoring. Inspected a Caterpillar generator system with a 750 gallon Custom Tank Technology double wall sub-base generator tank. Tank is equipped with a Pomeco spill containment device. Tank top normal and emergency venting are present on the primary tank. The tank interstitial (secondary space) is electronically monitored with an unknown mfg vertical mount level switch. Synthetic rubber fuel lines plumbed from tank top to overhead generator power unit. Anti-siphon protection provided by piping orientation. Overfill protection is provided by manual inventory procedures and hi & lo level alarm switches. Rochester inventory gauge provided for rough inventory readings. Inspected a second Cat generator system with a 3700 gallon JRS Custom sub-base generator tank. Morrison Fig 518 spill containment. Tank is equipped with a Krueger At A Glance inventory gauge for rough inventory information. Synthetic rubber fuel lines plumbed from tank top to overhead generator power unit. Anti-siphon protection provided by piping orientation. Tank top normal and emergency venting are present on the primary tank w/ emergency venting on secondary. An unidentified Hi level alarm switch provides overfill protection. The tank interstitial (secondary space) is electronically monitored with an unknown mfg horizontal mount level switch. Tanks are equipped with: -Normal venting. -Emergency venting for primary and secondary tanks. -Interstitial monitoring-Krueger leak gauges. -Electrical grounding present. -Exterior coating-appears in good condition. -Overfill protection-OPW 61 F Stop-1000 valves located in fills according to invoice dated 05/18/06. -Aboveground coated steel piping transitions to underground, 4¿ Ameron Dualoy 3000/LCX Coaxial Fiberglass (DEP EQ #291) piping runs from the top of the tank to remote dispensing/fill area. -STP located on northwest tank. -Solenoid valve on fill piping located below the fuel level, however the OPW 61 F Stop has drain vents which act as anti-siphon devices for fill piping. Solenoid valve for dispensing piping is located above the fuel level. Manual isolation valves are located at the top of the tank. Some rust beginning to develop on piping, recommend repainting. -(2) Piping transition sumps checked-dry. Secondary test boots are loose. -Remote fill within spill containment box-appeared dry and intact. Product label present. Tight fill and ball valve present. Spill box drain valve closed. Hose and nozzle-appear in good condition. -Suction pumps within secondary containment area-appear in good condition. Metal drain valve is closed. No obvious signs of leakage noted. Release Detection: Visual inspection of tank system and components, including open interstitial space. Release Detection Response Level Form is accurate. Site plan and photo's reviewed in FIRST. Monthly inspections detailed, timely and accurately reflect conditions found during inspection. Inspected 3 double wall steel tanks (Modern Welding UL 142) with dry spill containment, manual overfill procedures and interstitial monitoring. Tank Interstitials are dry. Tank #6 is still in place in grove at pump #6. #4 was moved to a new well on the west side of CR 731 between wells 1 and 2 in 2010. Tank #5 has been moved NE to the equipment barn on the east side of the rd and is currently in use as a vehicular fuel tank with a top mounted Fill Rite suction pump. Former location is clean with no evidence of petroleum staining in soils surrounding the slab. No assessment required. Inspected three 1K single wall steel tanks (Alabama Tank Inc., UL 142) in concrete block containments. Tanks and containments appear to be well maintained. Containments are covered, sealed, drained and secure. Aboveground synthetic rubber fuel lines on all tanks. Tank contents identified. Systems are corrosion protected. Facility has an ongoing containment maintenance program and patches cracks as they occur. Anti-siphon protection has been supplied on all pump tanks in the form of mechanical spring loaded check/anti siphon valves on both the suction and return lines. No financial responsibility. These systems must be placed out of service. Arrangements must be made to properly upgrade or close the systems. We were not provided with records of annual testing of the in-line leak detectors, the Veeder- Root TLS 350 certification or cathodic testing, monthly visual and electronic inspection, repair and maintenance of the systems. Description: 09/10/2010 Lindsay Brock & Alison Meetze (FDEP-SWD) met with Jerry Cox & Chad Engel (Sarasota County to perform a re-inspection. (2) 1,000-gallon {(1) containing 15W-40 multi-blend, new lube oil & (1) containing 40W new lube oil} and (1) 550-gallon (containing automatic transmission fluid}, Modern Welding, Glasteel II, double-walled, fiberglassclad steel USTs. The piping is single-walled, aboveground steel piping; where the piping goes underground, it is contained within single-walled, six inch, Ameron Dualoy 3000/L fiberglass piping (EQ-378). A re-inspection was performed for the purpose of accessing the room located in the bus wash building as there was no access available during the Department's annual compliance site inspection performed on 08/19/2010. The piping monitoring point located in the bus wash room was intact & dry (see photo). Aboveground, single-walled, steel piping located on the east side of the bus wash building has not been sanded or painted as of 09/02/2010 - sand and paint fuel piping and contact the Department to schedule a re-inspection.

Description: Arrived on site with Carlos Lincheta (DERM Inspector 1) at 9:00 am for a removal inspection of two 10000 gallon single-wall fiberglass UST's. I met with Contractor¿s Mike Quinn (PCC 052708) from Kinetik industrial Corp. (305-856-8283). I was told the following: The tanks were cleaned by CleanPro Enviro Solutions, LLC (Manifest # PP 0168), a total of 2300 gallons of petroleum contact water and 150 gallons of sludge were pumped out. The tanks were damaged during removal operations and will be disposed along with all the associated piping at local land field. The UST's were removed from the southwest corner of the subject property in an excavation size 30x40x11 ft and about a 2 ft of groundwater. Please note that only the north, west and part of the southern wall was visible during the inspection. No FFP, odor or sheen was noted in the groundwater as well as no staining was noted on the walls or the soil of the excavation that were visible. All monitoring wells around the tank farm are still on site. All excavated pea gravel will be taken off site and new pea gravel will be brought in for the installation inspection. Both tanks pieces were piled on site for visual inspection. TCAR must be submitted to the department within 60 days. Report was done in connected mode therefore no signature was collected. Report will be send via e-mail as per Mr. Quinn request. See pictures for more details.

Description: Fills Marked per API 1637: Yes, the tanks and spill containment are marked for their fuel. Overfill Prevention: Morrison Bros. 918 Clock Gauges with audible high level alarms. Additionally, the 20,000-gallon tank reportedly has a an OPW overfill protection valve. The alarm test button was pressed on the Morrison Bros. high level alarm panel and an audible alarm was heard. Spill Containment Condition: (3 Total) Clean and dry at the time of inspection. Tank Condition: Good, no leaks or corrosion observed at the time of inspection. Tank Interstice: All three were manually stuck at the time of inspection. The 20,000 and 14,000 gallon tanks were dry. The 12,000-gallon tank contained 3/4" of liquid. It was determined during the last compliance inspection that no violation or testing was required as long as the amount of liquid (reported as possibly condensation) remained the same and did not increase. Piping: Aboveground steel. The piping for the 20,000-gallon Diesel tank transitions from aboveground steel to underground double-walled fiberglass that leads to Dispensers #4 and #5. All aboveground piping was painted and in good condition with no leaks or corrosion observed. Equipped with Anti-Gravity Valves: Yes, solenoid valves on the dispensing lines and check valves on the fill lines. Piping Transition Sump: N/A, the d.w. piping interstice is monitored in the dispenser liners. D.W. Piping Interstice Open: Yes, in the dispenser liners #4 and #5. Dispenser Liner Condition: (7 Total) All were clean and dry at the time of inspection. Shear Valves Anchored Properly: Yes, mounted and tight for all. Dispenser Hose Condition: All were in good condition with no leaks or cracking observed.
